Benign cystic conjunctival melanosis is usually a bilateral condition with asymmetrical involvement and different morphological presentations.It should be differentiated from primary acquired conjunctival melanosis which is premalignant condition.Here we present a case of unilateral, multiple,well defined, pigmented cystic lesions in a 25 year old male patient who is a known case of juvenile open angle glaucoma in right eye operated with trabeculectomy with mitomycin C and left eye being absolute glaucoma. patient is on antiglaucoma medication in both eyes. Histopathological examination of excised lesion confirmed our clinical diagnosis as benign cystic conjunctival melanosis.This presentation is rare for being unilateral and with multiple cystic changes.
