Sympathetic ophthalmia (SO) is a bilateral diffuse granulomatous intraocular inflammation that occurs in most cases within days or months after surgery or penetrating trauma to one eye .Sympathetic ophthalmitis is itself a very rare pathology with an incidence, post penetrating trauma being 1.9% and 0.007 % after intra-ocular surgery.
METHODS: We report a case of 52 year male patient who developed sympathetic ophthalmitis following therapeutic penetrating keratoplasty (TPK) in the exciting eye following mycotic keratitis. The sympathizing eye presented with mutton fat Kps , disc edema and exudative retinal detachment. The patient responded well to IV Methylprednisolone and oral steroids.
CONCLUSION: SO presenting after TPK for fungal keratitis is a rare occurrence but if detected early can be managed effectively.
