Patients presenting with distal and common canalicular blocks undergo canaliculo dacryocystorhinostomy which carries the unavoidable drawbacks of prolonged surgical time, increased risk of bleeding and a visible scar.In an attempt to overcome these, we used the canalicular mini trephine which is a 21 gauge stainless steel tube with 0.81mm outer diameter and 16 mm length. It is provided with a plastic handle and hub and an intraluminal stylet. After removal of the obstruction with the trephine, a self-retaining mono canalicular stent made of silicone is left in situ for a period of 3 months. On follow-up for a period of 6 months, there was an anatomical patency in 83% and functional patency in 66.6% of patients in a study done on a total of 15 patients with distal canalicular blocks. This procedure has the advantages of it being minimally invasive, takes lesser surgical duration and there is no visible scar postoperatively.
