Aim: To report a rare case of bilateral MSK
Method: Case report
Result: A 66-year-old male, previously treated for Hansen’s disease, presented with redness and corneal infiltrate in OU for the past 6 months. On exam, there was a deep stromal whitish corneal infiltrate in OD in the visual axis. OS had a smaller whitish corneal infiltrate. Corneal scrapings taken from OU were positive for microsporidia on KOH. He was started on 1% Propamidine and 0.02% PHMB eye drops in OU. In view of worsening in OD, therapeutic penetrating keratoplasty was done. Histopathology confirmed spores on GMS. At 1 year of follow-up, there was no recurrence. Left eye healed with scarring.
Conclusion: Bilateral stromal microsporidosis keratitis is rare. Penetrating Keratoplasty is needed in cases refractory to medical management. Hansen’s disease may be a risk factor.
