Inferior Oblique overaction has always been an enigma for most of the ophthalmologists around the world. Various surgical techniques have been tried with limited success rate including myotomy, myectomy, ant transposition and ant and lateral transposition of inferior oblique muscle.
32 eyes of 20 patients underwent inferior oblique muscle lenghtening by Z myotomy along with horizontal deviation correction. Overaction of IO muscle was graded clinically and accordingly 1/2 to 2/3 IO muscle was cut from both border leading to weakness and lenghtening of IO muscle.
This procedure showed 90 % success rate with complete collapse of IOOA post operatively.
IO muscles Z myotomy is a safe,quick,suturless, simple and effective technique with short learning curve.
