Retrospective chart review of 21 cases was performed to assess outcomes of DALK in active acanthamoeba keratitis. Mean age of patients was 32+/- 10 years. Ulcers at presentation had mean (+/- SD) largest diameter of 7 (+/- 1.2) mm. Before surgery, topical anti acanthamoeba medication was administered for median duration of 57 days (range – 4 to 210). Graft size ranged from 7.5 to 11.5 mm (mean+/- SD = 9.26 +/-1.1). Big Bubble was achieved in 8 (38%) cases. Post operatively, descemetopexy was needed in 8 (38%) cases. Other complications included recurrence (2), bacterial infection (3), PED (2), stromal rejection (2), sterile melt leading to evisceration (1), HSV endothelitis (1) and interface vessels (1). Median follow up duration was 11.5 months (range – 1 to 78) during which 6 grafts (29%) failed and cataract surgery was performed in 7(33%) cases. Overall, 10 (48%) patients achieved BCVA > 20/60 at last followup.
