Scleral fixated intraocular lenses (SFIOLs) are widely used for surgical correction of aphakia in the setting of inadequate capsular support. SFIOLs are preferred over iris fixated intraocular lenses (IOLs) or anterior chamber IOLs when repeated pupillary dilation may be required post-operatively. Of the many techniques employed for scleral fixation of IOLs, the technique introduced by Gabor which involves exteriorizing the haptics of a three piece IOL and placing them within pre-formed partial thickness scleral tunnels concentric to the limbus is popular owing to its simplicity & good post-operative results. Surgeons, often during the initial stages of their career, experience a few difficulties in performing this technique. This video presentation aims at demonstrating the common difficulties encountered by the surgeon and the corrective measures that are necessary to achieve optimal IOL positioning by efficient intra-operative manipulation employing minimal surgical maneuvers.
