In this video surgeon has described his journey to deal with large Macular hole.
Before Mishelwaska, large Macular hole closure rate was very less and “U” shaped closure was not possible in most of the case. But after her paper on Inverted ILM flap technique, ” U” shaped closure rate drastically increased.
In the first part of the video surgeon described inverted ILM flap technique and autologous ILM transplantation.
But during this surgery it was very difficult to keep the ilm flap over d hole during last part of fluid air exchange.
So he used low molecular weight viscoelastic as a temporary tamponade to retain the flap
But becaz visco was transparent,later he has given it blue color by a mixing a drop of brilliant blue to take out fully at last part of FAE.
In last surgery surgeon has shown a novel technique ” combination of chromovisco tampo to do Macular hole surgery with minimal inverted ILM flap technique”
The hole closure rate is very High with inverted ilm flap technique.
