Aqueous shunt implantation often offers the best chance of long-term success in refractory glaucoma following vitreoretinal surgery.With advances in modern vitreoretinal surgical techniques glaucoma surgeons encounter encircling bands more frequently than the wider and more cumbersome silicone tires or sponges used earlier in scleral buckling procedures. Circumferential silicone tires or sponges are rare,retinal surgeons prefer segmental scleral buckle in the quadrant of retinal break and a 2.5 mm wide number 240 encircling silicone band to support the vitreous base.Location and height of the encircling band are crucial considerations in deciding where the episcleral plate of the aqueous shunt is inserted.In this video the authors wish to highlight certain operative techniques relevant to those eyes with pre existing encircling bands that require an aqueous shunt implantation.
