A 24/F presented with mild blepharoptosis in OD with a 2mm lift in phenylephrine test. We describe a novel surgical technique of performing a modified conjunctiva-sparing open sky mullerectomy. Surgical technique involves careful dissection and separation of muller’s muscle from the underlying conjunctiva under direct visualization. Subsequently, we successfully illustrate conjunctiva sparing subtotal mullerectomy. The stump of Muller’s muscle is then reattached to the superior tarsus with buried 6-0 silk. Conjunctival closure was achieved with 7-0 vicryl. Post-operatively, the patient has good ptosis correction with excellent levator function and lid contour. Our educational video demonstrates that conjunctiva sparing open sky mullerectomy can be performed safely with good success rate and cosmetic outcomes. This can be instrumental in training residents and fellows as this technique is quite effective and easy to learn.
