A 9-year-old boy presented with gradually increasing swelling in right eye for 3 months. History revealed multiple ocular surgeries following open globe injury 2 years back. Unaided visual acuity (VA) was 5/60 with well defined conjunctival cyst (16*10 mm) with aphakia and well attached retina. Intraoperatively, Ultrasonic Biomicroscopy was done. PCIOL was placed in sulcus. Localized peritomy along with blunt and sharp dissection helped in removing surrounding adhesions. Cyst was decompressed using 26-G needle. Cyst wall was then excised and marsupialization done with peritomy closure. Postoperatively VA was 6/24 with well apposed conjunctiva and stable PCIOL. Conjunctival cysts have a high rate of recurrence so instead of simple aspiration of cyst through an intact conjunctiva, the video demonstrates a technique of fine dissection and marsupialization of cyst wall preserving healthy conjunctiva to minimize complications and reduce recurrence.
