VT219 : Needle Revision Of Failed Filtering Bleb With A High Dose Of Injectable Mitomycin C

Failed filtering bleb are common problem after trabeculectomy.Revitalizing a bleb with a needle in the OR avoids the risks of invasive surgery as well as the need for daily medications. A mixture of 0.1 mL of mitomycin(0.4mg/mL) mixed with 1% xylocaine is injected with tuberculin syringe. TB syringe with a 27-gauge needle subcon-junctivally to the tight Tenon’s capsule around the bleb puncture the adherent tissue, and lift the scleral flap we carefully move the needle from side to side, breaking episcleral adhesions, until the bleb is re-formed. In a pseudophakic eye, the needle can be passed under the scleral flap, into the anterior chamber. a paracentesis can be made into the clear cornea, andthe anterior chamber can be filled with additional BSS, thuselevating the bleb further This video demonstrates simple technique of bleb revision with high dose injectable MMC.
