Purpose : To demonstrate the use of tarsal switch levator resection via anterior approach for treatment of myopathic ptosis in patient with poor eye protective mechanism.
Method : Tarso-conjunctival- levator graft was harvested via anterior approach from the upper eyelid. Size of autogenous graft was based on amount of desired elevation. Graft was transferred to the lower eyelid and sutured between the inferior tarsus border and free edge of conjunctiva.
Posto-perative: Post operatively both eyelids elevates at a desirable amount and unmasking of visual axis in primary gaze.
Conclusion: Tarsal switch procedure is effective technique in opening the visual axis in primary gaze and decreasing the risk of postoperative exposure in patient with poor eye protective mechanisms.
