We preset a case of 25 year old high myopic male. OD BCVA was 6/18 with -12.5DS -1.5DC 180degree and OS BCVA 6/12 with -10 DS -2.0DC 180degree. VISIAN Toric ICL (STAAR SURGICAL) implantation was planned in OU. During OD surgery on injecting the lens inside the eye it opened in an inverse manner with optic nearly touching the anterior capsule. Following which ICL was explanted manually through the same 3.2mm incision and injected again. Despite the correct loading of ICL in the injector cartridge system again it opened in inverse manner. Third time while injecting the ICL, surgeon rotated his hand clockwise and anticlockwise according to the direction of unfolding of the lens and it was correctly placed. Postoperatively day 1 patient had corneal oedema which resolved till day 7 and patient gained UCVA of 20/30. Our video demonstrates management of this known complication of toric ICL with good postop results.
