Retinal manifestations like flame shaped, dot blot and subhyaloid haemorrhages, Roth spots, retinal pallor and disc edema are seen in moderate to severe anaemia. Bilateral macular involvement in case of anaemic retinopathy is rare. A 16-year-old girl, admitted under Medicine department for anaemia was referred to Ophthalmology OPD due to sudden painless diminution of vision in both eyes. On examination, BCVA was 6/36 in both eyes. Fundus examination showed multiple pre retinal haemorrhages, few Roth spots and pallor of the posterior pole. Bilateral dense subhyaloid haemorrhage in macular area was seen. Investigations revealed moderate degree of dimorphic anaemia. Patient was treated with packed red cell transfusion and iron supplementation. Vision improved gradually over 12 weeks to 6/6 with complete resolution of subhyaloid haemorrhages. Fundus changes seen in moderate to severe anaemia can compromise vision. Hence a thorough ocular examination is mandatory in such cases.
