Aim:
To study the management of secondary glaucoma in case of microspherophakia.
Method:
A 37-year-old one eyed male presented with acute attack of angle closure glaucoma with pupillary block secondary to isolated microspherophakia, anteriorly dislocated cataractous lens. Considering other eye being absolute, He underwent lens extraction with anterior vitrectomy and surgical peripheral iridectomy in LE.The patient`s best corrected snellen’s visual acuity improved from FCNF to 3/60 and intraocular pressure (IOP) decreased from 50mm of Hg to 18 mm Hg without continuation of any antiglaucoma medication.
Conclusion:
This case demonstrates that microspherophakia should be ruled out even in middle aged patients presenting as acute angle closure glaucoma. Lensectomy is the first line management of these patients with satisfactory recovery of visual acuity and IOP control.
