Purpose: To discuss rare and confusing presentation of a case of Atypical optic neuritis
Case Report: 56 years old male presented with compliant of left eye redness for 6 months. BCVA was 6/6 in RE, 6/9 in LE. Examination revealed grade 1 RAPD in left eye and blurred disc margins. HFA showed superior arcuate scotoma and on FFA disc leakage was present. VEP showed reduced amplitude and P100 prolonged latency. With diagnosis of atypical optic neuritis Injection methylprednisolone followed by oral steroids were given as per ONTT. But disc edema persisted for which he was kept on close follow-up. Patient presented one month later with painful loss of vision in other eye. Diagnosis of viral keratouveitis was made and antivirals were started. Surprisingly BCVA improved to 6/6 and disc edema in fellow eye disappeared after 2 weeks .
Conclusion: 1)Optic neuritis can have varied presentation.
2) The clinical presentation and response to treatment outweighed the investigations.
