The ocular manifestations of syphilis are wide -ranging. Ocular syphilis can virtually affect any part of the eye and can occur during any stage of infection. The syphilitic retinitis can mimics retinitis caused by various viral etiologies thereby making the diagnosis a difficult task. In this report, we describe a 28-year-old female with diminution of vision following road traffic accident. On fundus examination, there was optic disc pallor with multiple dot blot and flame shaped subretinal haemorrhages. The rare and unique finding in the fundus was that there were two D-shaped pre-retinal or subhyaloid haemorrhages. Quick initiation of intravenous antibiotics led to good visual outcome for this patient despite significantly decreased visual acuity on presentation.
