50 year old female came with deviation of both eyes associated with limited eye movements bilaterally since childhood, progressing over past 10 years, Right eye has total restriction of movements and Left eye marked limitation of movement, more on lateral gaze and upgaze. No history of ptosis. 1 out of her 6 siblings have same complaint. Vision in right eye – finger counting close to face, densely amblyopic & left eye pseudophakic 6/36. On examination, face turn to left with chin elevation was noted with both eyes fixed in esotropia. Extraocular movements were restricted in all directions in the right eye and elevation, depression and abduction restricted in the left eye. Cover test and prism bar cover test not elicited due to movement restriction. Anterior segment normal with high myopic fundus in both eyes. Forced duction test in both eyes- positive.
