58 year old male presented with a history of a black gray spot in his right eye since 3 weeks .Left eye was normal.He gave history of flu like illness 1 month back.Visual acuity in right eye was 6/9. Fundus showed paracentral, well demarcated, wedge-shaped opacification nasal to fovea.Redfree picture confirmed the lesion.Spectral domain OCT through the lesion suggested hyperreflective band that involved the Outer plexiform layer &Outer nuclear layer with associated disruption of the IS/OS &OS/RPE layers. These findings were consistent with the diagnosis of a rare variety of Acute macular neuroretinopathy – Type 2 lesion Paracentral acute middle maculopathy. 2 months later The SD-OCT revealed that the hyperreflective area had resolved.Occlusion of deep capillary plexus (type 2) located in the outermost portion of the inner nuclear layer cause this pathology.Diagnosis of rare pathologies like acute macular neuroretinopathy has significantly improved with the use of spectral domain OCT.
