Case report: A 33 year old female presented with squinting of left eye since birth. On examination patient had large angle hypertropia with exotropia in left eye which was a diagnostic challenge. There was no craniofacial anomaly. Imaging showed reduction in volume of inferior rectus muscle in left eye. Intraoperatively inferior rectus muscle was found to be absent. Inferior oblique anteropositioning along with superior oblique tucking was done. Superior rectus and lateral rectus muscle recession was done. The procedure resulted in excellent alignment in primary position. Hence high index of suspicion is required to consider inferior rectus aplasia. Magnetic Resonance Imaging plays a vital role in such diagnostic dilemma. This case of unilateral inferior rectus aplasia without craniofacial anomaly is presented for its rarity and surgical expertise to achieve the best possible outcome.
