Exotropia is known to have fair surgical success, often needing multiple surgeries for satisfactory outcomes. We studied 30 patients with different types of exotropia, who underwent surgery with a follow-up of ≥3 months. The 1st post operative day outcome was compared to that at 3 months. We divided our patients into 3 groups- intermittent exotropia (group 1), constant exotropia (group 2) and sensory exotropia (group 3. In group1 and 2, patients with orthotropia or flick esotropia (<5 PD) showed recurrence at 3months. However patients with small esotropia(5-10 PD) showed satisfactory alignment at 3 months. In group 3, patients with orthotropia immediately post surgery continued to have satisfactory alignment at 3 months than those with a flick esotropia. We conclude that for sensory exotropia, orthotropia should be the desired end point and for all other exotropias, small esotropia should be the desired end point on the 1st post operative day to achieve satisfactory results over time.
