A prospective study was conducted to determine the role of OCT – RNFL and other preoperative factors such as optic atrophy, tumour size, visual acuity, visual field defect and duration of symptoms, in determining the visual prognosis of patients with pituitary macroadenoma undergoing trans-sphenoidal resection. Out of the 58 subjects, visual symptoms were present in 53.4% (N=58) and 49.13% had optic disc pallor. The mean visual acuity, mean deviation and visual field index were 0.32 LogMAR units, -11.31db and 72.84% respectively. Analysis of subgroup that followed up showed that 53.3% had an improvement in visual acuity. Among those with optic atrophy, there was a trend for thicker RNFL to have better visual outcome. There was no conclusive evidence that pre operative retinal nerve fibre layer thickness can predict post operative visual outcome. Pre operative visual acuity, depth of visual field defect, duration of symptoms and tumour size did not predict post operative visual acuity.
