Dr. JAYA VOHRA
Purpose: To report a rare case of lupus choroidopathy. Methods: History of present illness, clinical features, imaging and treatment outcome were studied. RESULTS: 25/F, a known case of SLE with nephropathy presented with bilateral blurred vision. She was on immunomodulatory therapy (IMT) and oral steroid 20mg. Her BCVA was CF in OU. Anterior Segment was normal. OU Fundus examination showed disc edema, exudative retinal detachment (ERD) and multiple subretinal lesions. OCT showed serous detachment, subretinal fibrin and choroidal thickening. FFA showed wedge-shaped choroidal filling defects suggestive of infarction. BP was within normal limits. Diagnosis of SLE Choriodopathy was made. Intravenous methyprednisolon followed by oral steroid was given along with OS subtenon’s steroid injection. ERD resolved and vision improved (OD: 6/36; OS: 6/18) over 4 months.
CONCLUSION: Choriodopathy is a rare manifestation of SLE which may develop despite IMT and may require intensive steroid therapy.
