To compare the efficacy of blade against crescent in constructing sclera tunnel of temporal SICS, in a community setup. Prospective comparative study of 100 cases, randomly assigned to tunnel made from crescent or razor blade (50 eyes each), by a single operating surgeon. Comparative analysis of structure and integrity of tunnel done.Of the 100 eyes operated, the mean average depth ,at section and at entry site was 70-72 microns deeper with blade compared to crescent .Self sealing wound was achieved in 49 of the 50 cases in both groups. Tunnel complications included DMD in 1 eye in blade group and premature entry in 1 eye in both groups.The time taken with crescent Vs blade was on an mean average 32.9 sec vs 27.3 sec.In terms of cost per instrument, razor blade costs 3rs Vs crescent costs 56rs .Tunnel entry needed no keratome with blade compared to crescent group.The razor blade is a safe, equally efficacious, and an economical alternative to crescent knife in a community setup.
