Down syndrome is trisomy 21 which results in ocular manifestations like refractive error, strabismus, developmental cataracts, Brushfield spots, glaucoma, keratoconus and retino-vascular anomalies. A 13 year old male with Down syndrome, referred to us for ophthalmic examination from Medicine OPD, was evaluated for visual acuity, pupillary response, ocular movements, ocular alignment, corneal light reflex test, cycloplegic refraction, indirect ophthalmoscopy and keratometry. The patient was diagnosed as Bilateral keratoconus with bilateral developmental cataract and was managed with SICS with foldable IOL implantation for cataract and later with rigid contact lens for keratoconus. He was advised for regular follow-up in which it was found that the visual acuity improved and is stable after 6 months postoperative period.
