Aim: To report a case of bilateral retrobulbar neuritis due to cytomegalovirus(CMV) infection in an immunocompetent patient with albinism.
Method:Case Report
A 26 year old male patient with albinism presented with fever since 5 days, bilateral sudden visual loss and retrobulbar pain since one day. BCVA was counting fingers at 1metre in right eye and 2metres in left eye with abnormal colour vision. Examination showed horizontal nystagmus, bilateral hypochromic irides and 4mm pupils with sluggish response. Fundus examination showed features of ocular albinism. Cerebrospinal fluid study reported CMV antibodies. MRI was normal. VEP showed delayed response in both eyes. Following therapy with IV Gancyclovir, and IV Methylprednisolone his visual acuity improved to 6/36 with normal color vision in both eyes within 2 weeks.
Conclusion: CMV infection rarely causes bilateral retrobulbar neuritis in immunocompetent patients. Prompt diagnosis and treatment may ensure complete visual recovery.
