Purpose:To report a case on Acute angle closure closure glaucoma with resolving Glaucomaflecken. Method: A 64 year old male, farmer, presented with acute pain and decreased vision in right eye since 1 week.Right eye had only HM+. IOP 54 mmHg and 21 mmHg in RE and LE respectively.AC was flat with diffuse pigmentation on posterior corneal and anterior lens surface.Signs-iris atrophy, glaucomaflecken: central dense white sub epithelial lens opacity.closed angle on gonioscopy. Results: Folowing treatment IOP reduced to 30mm Hg and resolved to normal limits in 3 days. Slit lamp-RE shows relief of the AAC. Reduced congestion and clear cornea.Resolution of glaucomaflecken occurred within 1 moth of follow up. Vision improved drastically to 6/6 post treatment.Conclusion: The spots have been seen only in the anterior sub capsular cortex, never posteriorly.
Further evaluation on mechanism of glaucomaflecken is important.
