Here we report a case of a 27 year old male who developed extensive retinal vasculitis which was attributed to HLA B 27 positivity. He came in with complaints of sudden loss of vision. Ophthalmological examination revealed extensive superficial hemorrhages in both eyes along with cotton wool spots and vascular sheathing with minimal vitritis. All other systemic etiological factors were ruled out. Patient was observed for resolutions of hemorrhages after which he underwent Fundus Fluorescein Angiography and Optical Coherence tomography. He was treated with oral steroids to control the inflammation. The anterior segment findings in case of HLA B 27 patients are well documented. This case is presented to emphasize the posterior segment manifestations in HLA B 27 positive patients.
