A 24 year old female came with history of swelling of her right eye and lids since 12 years coinciding with her menstrual cycle and lasting for 4-5days. She gave history of one cesarean section. On examination, the right eye revealed perioribital edema, chemosis, subconjunctival hemorrhage and mild axial proptosis with subsequent clinical resolution in the intermenstrual period. All the basic blood investigations were normal except for mild anemia. MRI Pelvis at the site of the cesarean scar showed a possiblilty of scar endometriosis. A diagnosis of Orbital Vicarious Menstruation was made conferring with the gynecologist and radiologist. Vicarious menstruation means cyclical bleeding outside the uterine cavity during the menstrual cycle.It is rare and very few cases involving the orbit have been reported till date.Treatment options include ovulation suppression through hormonal therapy or resection of aberrent endometrial tissue performed under the guidance of the gynecologist.
