A case of 30 years old female presented with diminution of vision in both eyes since one week.Anterior segment evaluation showed episcleritis ,keratic precipitates , flare 2+ ,cells 1+ and posterior synechiae in right eye. Left eye anterior segment was normal except pupil was sluggishly reacting. Fundus examination showed bilateral optic disc hyperaemia suggesting optic neuritis.Features of Cytomegalovirus(CMV) such as linear shape keratic precipitates ,target lesions on skin of hand and feet were present.All findings were documented and photographed.Patient underwent MRI (Brain and Orbit),HRCT of thorax,HIV Screening and others systemic examination.These investigation revealed Human Immunodeficiency Virus(HIV) infestation, pulmonary tuberculosis,thickened and swollen optic nerve,extensive pansinusitis with sino nasal polyposis. Thus, a meticulous clinical evaluation and appropriate investigations are warranted for appropriate management of such cases as a part of life saving measure.
