One of the unsatisfactory outcomes of cataract surgery is the insertion of an IOL implant of the wrong power. A biometry error is supposedly one of the most common cause. A 68-year-old man underwent uneventful phaco with PCIOL of the power +23.0D OD. One week post operatively the patient had unaided visual acuity of 6/36. After a month a spherical error of +3.00DS\+0.50 cylinder at 180 was recorded which resulted in BCVA of 6/12. He was dissatisfied with the surgery. He did not want to use glasses and requested the surgeon to explant the IOL , A repeat biometry revealed expected IOL power to be +25D .Patient was taken up for IOL exchange with a new IOL insertion of the above mentioned power. After an uneventful cataract surgery the patient had a post operative best corrected visual acuity of 6/9 with 0.5Dcyld 180. The pictures will be shown which were taken during this relatively simple yet important surgery.
