To present a case of 57 year old woman who came with complaint of diminution of vision in both eyes since one week. On examination fundus showed nonarteritic anterior ischemic optic neuropathy and severe nonproliferative diabetic retinopathy in both eyes with macular edema in right eye confirmed by fundus fluorescein angiography and optical coherence tomography. Her erythrocyte sedimentation rate was 105 mm at 1 hour, HbA1c was 10.33 and blood pressure was 200/100 mm of mercury. MRI brain was within normal limits. She was started on intravenous methylprednisolone and followed by oral steroid therapy with strict diabetes and hypertension control. After completion of steroid treatment there was improvement in vision in right eye only and fundus showing resolving nonarteritic anterior ischemic optic neuropathy in both eyes and resolving macular edema in right eye. But on further follow up vision diminished in both eyes with fundus showing pale disc.
