35 yr male, came to OP for severe pain, swelling, itching & blurring of vision in RE 3 days after bee sting. On S/L exam a retained bee stinger (Ovipositor) was found in cornea at 7 ‘O’ clock position, corneal edema, hypopyon & features of iridocylitis. Under topical anesthesia Bee stinger was removed.VA was reduced to 6/60. Topical Antibiotic, Antifungal, Mydriatic & steroid ie. Natamycin hourly x2week, Atrochlor-D 12hrly x 2week ,were instilled topically along with Tab. Cefixime 200mg twice daily x10 D, Cap. Fluconazole 150mg daily x 10 D,Tab. Aceclofenac with seratiopeptidase twice daily x 1wk given orally. After 1wk corneal edema was markedly reduced. After 2 wks all symptoms &signs subsided leaving a small nebular corneal scar in the retained stinger area & VA improved to 6/12. Pt responded well to the treatment. Corneal bee sting is a rare ocular trauma but can result in sight loss complications hence prompt & proper treatment can prevent corneal blindness.
