Purpose: To compare outcomes of Corneal Stromal Augmentation with DALK in advanced non-scarred keratoconus
Methods: Femto assisted donor stromal lenticule (140-200 mic thick,7.5-8.5mm in dia.) was fashioned, and a bed created intrastromally with 2 incisions 180° apart in the keratoconic eye, through which the lenticule was transplanted
Results:15 eyes in both. Stromal augmentation and DALK improved vision by 4 & 2 lines. Both increased mean CCT to normal. Both flattened keratoconus significantly. DALK flattened anterior cornea (p=0.178), Augmentation flattened posterior cornea more (p=0.001). Both reversed irregular astigmatism (p <0.05). Post OP S/I asymmetry closer to an assumed acceptable difference of 2D in both Conclusion: Stromal Augmentation, in terms of improvement in vision, flattening of cornea, reversal of ectasias, and improvement in irregular astigmatism was comparable & no worse than DALK, without sacrifice of recipient tissue and suture related events
