Modern cataract surgery aims to provide spectacle free life to the patients. As a result, clinicians have to be spot on with their IOL power calculations. To add to the complexity, we have an increasing number of cataract patients with past history of incisional/laser refractive surgery. Owing to the changes induced by the refractive surgery, certain assumptions about the ocular structure; made in older IOL formulae; do not hold true and unless corrected for can lead to significant post op deviations from the expected or intended outcome. Some newer and hitherto unattended concepts pertaining to ocular biometry e.g. Effective Lens Position, Equivalent Keratometry Reading, Corneal Power, Customised A constant etc. have become important in the above backdrop. For most cataract surgeons these concepts remain an abstract ophthalmic jargon. In this IC we would visit each of these concepts with background information and would elucidate their importance with hypothetical case scenario.
