FP936 : Multifaceted approach in the management of peripheral ulcerative keratitis

Peripheral ulcerative keratitis(PUK) can be potentially sight threatening especially if not treated adequately. Timely intervention in the form of accurate clinical diagnosis, systemic evaluation& treatment in association with immunologist can give rewarding results. We present a case of PUK in 34 year male who came with complaints of pain, redness, watering in left eye since 3 months. On examination, patient was clinically diagnosed to have peripheral ulcerative keratits with impending perforation. A detailed systemic evaluation was done to look for underlying systemic pathology and patient was referred to immunologist and was started on oral steroids and systemic immunosuppressants (methotrexate). Amniotic membrane grafting was done in the acute phase to prevent perforation and patient was followed up closely. He improved dramatically with multipronged treatment approach. This case highlights the need for immune therapy in addition to topical treatment for ideal outcomes.
