Surgical management of Giant retinal tears sparks controversies even now. Although Giant retinal tear accounts for only 1.5% of all rhegmatogenous retinal detachments but it has the highest rate of recurrences (45%).Perfluorocarbon liquids (3rd hand of VR surgeon) has revolutionized GRT surgery but still the visual outcomes are poor. Adequate vitreous base and anterior flap excision ,the prime determinants of surgical success demand lensectomy in majority of cases adding to significant visual morbidity. This video illustrates a lens sparing surgical technique using CHANDELIER ENDOILLUMINATOR. Chandelier endoilluminator illuminates the peripheral retina to allow safe anterior vitreous base excision.It also allows the surgeon to indent external sclera to shave the anterior vitreous and to extend laser burns upto ora.Owing to its utmost utility during complex retinal surgical procedures we have termed Chandelier endoilluminator as Vitreo-Retinal surgeon’s 4th hand in GRT surgery.
