Drop less cataract surgery is a new concept where steroid and antibiotic are introduced transzonularly into the vitreous cavity at the end of IOL implantation in phaco surgery. This eliminates the need for instillation of eye drop post operatively .0.1ml each of Moxifloxacin eye drop and Triamcinolone acetonide injected transzonularly at the end of IOL implantation with the help of 27 G curved cannula. OVD injected into the ciliary sulcus space to make room for easy introduction of the cannula. The IOP rise following the injection is counter balanced immediately with bimanual IA. The whitish mixture is clearly seen floating in the vitreous cavity. The eye is shaken bimanually to distribute equally in the vitreous cavity.We have done a study where this procedure was applied in 60 eyes. >6/9 UCVA achieved in 95% eyes at the end of 2 months period. There was normal IOP in all examinations during that period. This video demonstrates the procedure with intra op complication in 8 cases.
