Purpose: To video demonstrate a unique femto cataract complication and teach ways to overcome
Methods: A 65 YO female,diabetic with grade IV cataract & a semi dilated pupil, underwent femto cataract procedure. After docking, pupil autorecognition was successful,and a 5.25 mm capsulorhexis initiated; during which more than 180° of iris was caught within rhexis,resulting in lasers on the surface of iris parallel to pupil,further leading to irritative miosis. Results: False pupil auto recognition occurred (a reflected artifact sensed as pupil,resulting over sized).Lasers fired on iris, resulting in hyper reflective shadow of iris pigment release on OCT. Following learnt. Mark Pupil manually in doubt. Caution on Iris involvement in capsulotomy. Watch release of pigments as hyperreflectivity on OCT.
Conclusions: Though Femto optimizes cataracts, an eagle eyed approach to adverse events are essential. Human skill outweigh advanced systems in its management.
