A 60-year-old female, known case of Fuch’s endothelial dystrophy, presented with spontaneous posterior dislocation of in-the-bag brunescent cataractous lens in right eye. Standard 25G pars plana vitrectomy was undertaken. Nucleus was freed from all vitreous attachments. Triamcinolone assisted posterior vitreous detachment and vitrectomy was done with thorough shaving of vitreous base. Emulsification was avoided due to hardness of nucleus and compromised endothelium. Perfluorocarbon liquid (PFCL) was used to float nucleus upto pupillary plane. Dispersive viscoelastic was injected into anterior chamber and nucleus was delivered through limbal incision. Retinal periphery was examined before PFCL removal and wound closure. PFCL assisted delivery proved simple, time saving and prevented complications of excess phacoemulsification like endothelial compromise, retinal toxicity and scleral burn. Thorough vitrectomy, viscoelastic cover and periphery screening is essential in such cases.
