This mentally handicapped patient with dense cataracts required general anesthesia for exam and treatment. To restore vision for a lifetime with one surgery after EUA the dictates of the international Society of Bilateral Cataract Surgeons were followed, the author’s technique of circumferential disassembly with cross action chop enabled efficient safe phacoemulsification, posterior capsule was opened, Berger’s space defined with OVD protecting the hyaloid and a continuous rhexis performed (per Gimbel). A 3piece acrylic lens was buttonholed into Berger’s space (per Menapace). After intracameral moxifloxacin and indirect ophthalmoscope, re-prep and drape, the same technique was used for the slightly better contralateral eye.
