Abstract: the video shows a challenging phaco in an advanced cataract in a hypermetropic eye with a small Pupil and atrophic iris . During the course of removal of epinuclear material there is an inadvertent injury to the iris with the second instrument with copious bleeding in the bag and anterior chamber. Situation was stabilised with gentle bimanual irrigation and aspiration . Difficulty in placement of a foldable multi piece IOL through a cartridge was shown .
Ultimately stable IOL , stable AC , clear cornea!
