Purpose- To demonstrate a very innovative way of doing Capsulorhexis by Phaco:Irrigation-Aspiration
Method :- After Capsular staining,a small initial capsular flap is made with cystotome needle.I\A rate at15-20cc\min,Vacuum at 60-100mmhg,and bottle height is adjusted at 100-110cm on Phaco machine.Irrigation canula with continuous irrigation is inserted from a sideport to form anterior chamber and a aspiration canula is inserted from another sideport.The initial flap is aspirated and the hold is maintained with vacuum and simultaneously tearing force is applied to achieve desired capsular tear. The flap is released and re-grasped with the help of Re-flux mode to achieve desired ccc.
Results :- CPIA is very useful in Morgagnian cataract as the liquified cortex getting aspirated simultaneously and you get a clear view of flap and surgical field and also capsular tear doesn’t require stable background support which is not there in Morgagnian cataract.
