Background : An idiopathic full thickness macular hole generally develops in an elderly population.
Case : A 19 year old one eyed boy who came for routine check-up diagnosed with full thickness macular hole with visual acuity 6/18.
Observation : Having denied history of trauma to the right eye, fundus of the right eye showed a full thickness macular hole with surrounding halo. There was no evidence of posterior vitreous detachment, peripheral retinal pathology or vasculitis. Neither a macular hole operculum nor macular traction was detected. Visual acuity was 6/18 for the following 3 years. Opthalmoscopic examination of the other eye showed phthisis with no perception of light since ten years following trauma with tennis ball.
Conclusion : Although the etiology of the macular hole in the present patient is unclear, unknown trauma might have been responsible for formation of macular hole in the right eye.
