Reporting a rare case of adenocarcinoma rectum with orbital metastasis. Only 7 cases reported world wide. 40 year old man presented with pain,redness,defective vision RE with weight loss. O/E RE vision- no PL, proptosis, iris neovascularisation, IOP 48mmHg, media hazy due to vitreous exudates. LE vision 6/6, localised exudative retinal detatchment in infero temporal quadrant. Ultrasound B-Scan showed RE-total retinal detachment. Investigations revealed RE- Retro-orbital mass,left lung apical lobe lesion and rectal mass. Biopsy from rectal,orbital and lung lesions showed adenocarcinoma rectum and metastasis to lung and orbit. Diagnosis of adenocarcinoma rectum with orbital and lung metastasis confirmed
