A 54 years old gentleman with history of Type 2 DM for the past 15 years was diagnosed with early PDR with DME and was posted for FFA. Color fundus photographs taken before injection of dye showed a small patch of neovascularization elsewhere (NVE) in the supero-temporal region in the left eye. As soon as the dye was injected, patient had an episode of sudden violent retching and vomiting. It took around 2 minutes to stabilize the patient following which acquisition of images was again initiated. While reviewing the images, a round mass with punctate hyperfluorescence surrounded by a ring of intense hyperfluorescence was noted in the left eye in the supero-temporal region corresponding with NVE. Color fundus images were again acquired from the same region of the left eye that showed a subhyaloid bleed. After about a half hour, patient complained of sudden onset decrease in vision. Fundus picture at this stage revealed breakthrough bleed in vitreous cavity covering the macular region.
