Purpose-To report a case of presumed endogenous endophthalmitis.Materials&methods:39 year old male,k/c/o of seminoma testis,chronic kidney disease on dialysis,hypertension came with complaints of blurred vision in right eye.No history of ocular surgery,trauma.Patient was febrile.Visual acuity right eye-counting fingers.Examination-cells in anterior chamber,vitreous haze,large subretinal abscess at macula with superficial haemorrhages.Left eye-normal.Blood and urine cultures were negative.A diagnosis of presumed bacterial endogenous endophthalmitis was made and intravenous antibiotics-Meropenem,Linezolid &Metronidazole were started.Results Resolution of lesion & clearing of vitreous haze within 3 days of starting treatment.Visual acuity improved to 6/60 on day 7.At end of 3 weeks visual acuity was 6/9 with fundus revealing a small scar.Conclusion-Early diagnosis & prompt administration of intravenous antibiotics can greatly improve prognosis without recourse to intravitreal antibiotics
