Polypoidal choroidal vasculopathy is a well known cause of central vision deterioration and is characterised by recurrent subretinal bleeding. It is not infrequent to find massive submacular haemorrhage (SMH) in these cases, wherein the visual prognosis is usually poor. The case discussed here highlights rare presentation of massive SMH with good visual acuity, wherein the patient had complaints of a mildly blurred vision. The presence of minimal subfoveal bleed and only sub-pigment epithelial hemorrhage explains the preservation of vision in our case. The case was managed with anti-vascular endothelial growth factor (VEGF) injection leading to visual recovery.
