Purpose : to analyse the therapeutic efficacy of intracameral Amphotericin B in deep fungal ulcer Cases in RIMS, Ranchi.
Method : 22 patients( 22 eyes) diagnosed with deep keratomycosis, having >1/2 stromal depth involved, marked epithelial defect and irregular infiltration were included. Diabetic, immunocompromised, lid defects, steroid users were excluded. Baseline mean VA 0.467(log MAR). Mean epithelial defect 5.21+/-0.8 mm and stromal infiltration 5.48+/-0.6 mm. organism retrieved predominantly Candida sp.Each eye received 10 mcg AmB(0.01 ml) intracamerally on 1st day, ulcers were evaluated every 48 hours, repeat injections required.
Results : 13 eyes (single injection) had rapid ulcer healing by 5th day. 2 faced evisceration. Rest 7 Cases evaluated up to 18 days (mean 2.6 injections). Full clinical recovery gained by loss of hypopyon, epithelial defect closure, clearly defined corneal opacity.
Conclusion : IC AmB showed higher efficacy and faster response treating deep keratomycosis.
